On the grounds of patient safety we will not take any prescription requests over the telephone.
HOW CAN I REQUEST MY PRESCRIPTION?
Post or hand in your re-order slip (on the right hand side of your last prescription, tick the items you require), and put in drop-off box on the reception counter
Complete the written request form (available on the reception desk) and pop it into the drop-off box
Use our online prescription service at www.burnfieldmedicalpractice.co.uk ( the current repeat ordering service is ceasing in the New Year 2019 and being replaced with online prescription ordering and online booking. All on the website.
Pharmacy prescription service (contact your chosen pharmacy 1 week before you will need your medication. They will contact the surgery to arrange your prescription and then get your medicines ready for you to collect from the pharmacy)
WHAT IF I NEED MEDICATION NOT ON MY RE-ORDER SLIP?
Write the name, strength, dose and quantity of the medication(s) onto the re-order slip (the doctor will assess your request)
Request through on line ordering.
WHAT IF I DO NOT HAVE A COMPUTER AND CAN’T GET TO THE SURGERY?
PLEASE ALLOW A MINIMUM OF 24hrs BEFORE COLLECTING YOUR PRESCRIPTION REQUEST AT THE SURGERY.
FOR PRESCRIPTIONS SENT DIRECTLY TO THE PHARMACY, ALLOW 3 DAYS FOR YOUR MEDICICNES TO BE READY FOR COLLECTION AT THE PHARMACY.
IF YOU NEED YOUR REPEAT MEDICATION URGENTLY AND CANNOT WAIT UNTIL THE NEXT DAY FOR YOUR PRESCRIPTION YOU SHOULD ASK YOUR PHARMACY FOR AN URGENT SUPPLY

CHILD FLU INFORMATION
Children 2, 3 and 4 years also 5 year olds who are not yet in School.
For most children, the flu vaccine is given as a nasal (nose) spray into each nostril. It is not an injection. It’s quick and it’s painless. There’s no need to sniff or inhale the vaccine; only a tiny amount is sprayed into each nostril.
An alternative form of the flu vaccine may be suitable for children who cannot have the nasal spray vaccine. These children will be offered a flu vaccine as an injection in the upper arm.
