Permanent Registration
If you live within our practice boundary and wish to register, please complete the registration forms below and email to nhsh.gp55889-reception@nhs.scot at least one day before any appointment is required.

Please be advised if you have not been registered with a GP in the UK we will require documentation and identification.

Online Services
Please complete the following form to allow us to set you up for online patients services for prescription ordering.
Temporary Registrations
If you are visiting the area and require GP services and you are staying within the practice catchments area you will be able to register with the practice as a temporary resident.
This service allows patients on holiday or visiting family within the area to be able to registered with a GP practice and receive temporary clinical care for a maximum three month period (although this can be extended depending on the clinical care you may require). At the end of your stay all clinical information is then transferred to your own GP keeping your patient record up to date.
We offer a wide range of Private Non-NHS services to arrange an appointment contact reception on 01463 220077.
Payment will be taken at the time of booking the appointment. If you are not able to make payment at time of booking, you will have 3 days to arrange payment to secure your appointment. If payment is not received within 3 days the appointment will be cancelled.
Once payment has been taken we will ask for a private registration form to be completed prior to your appointment.
If you need to cancel your appointment email nhsh.gp55889-reception@nhs.scot with your name, D.O.B, appointment date & time and the same card information that was used to make payment to enable us to process a refund.
